Family Care / IRIS
The ADRC shall assure that all consumers are informed about eligibility for Long-Term Care Funding. The ADRC shall administer a Long Term Care Functional screen to determine functional eligibility for Long Term Care programs, such as IRIS and Family Care, and will provide information regarding financial eligibility guidelines.

- High Phone Bills
Local Telephone Service providers are required to give discounts to residents who meet certain eligibility requirements. You need to call Customer Service at the Telephone Service Provider, and request an application form. After you fill it out and mail it back, the provider will confirm the information given, and will then credit your account between $5.25 and $7.00 each month. The Wisconsin Homestead Tax Credit program is designed to help low income residents who are paying real estate property taxes directly through a property tax bill, or indirectly in the form of rent. The amount of the Tax Credit that you may be eligible to receive is determined according to the amount of your income in relation to the amount of property tax (or rent) that you pay.

The Low Income Energy Assistance Program is designed to help residents with low income to receive a credit on their fuel and electric bills thereby reducing the amount they have to pay for the season.

- Adult Family Homes - Kewaunee County, Manitowoc County
A place where 1 to 4 adults who are not related to the operator reside and receive care, treatment or services that are above the level of room and board.
- Residential Care Apartment Complex (RCAC) Kewaunee County, Manitowoc County
This is an apartment type setting where five or more individuals live and offer independence and control over personal space, care decisions and daily routines. This type of housing would benefit someone who needs some assistance with daily living care and nursing services yet can function in an apartment setting. Up to twenty-eight hours per week of supportive services such as assistance with medication, bathing, dressing, grooming and nursing care are provided. If a resident requires more than twenty-eight hours of supportive service per week, the tenant can contract with a home health care agency or obtain assistance through informal supports such as friends and family to assist with care needs. Emergency assistance is available twenty-four hours a day. Social and recreational activities as well as meals will be available. Be sure to ask about the services and if there is an additional charge for any of them.
Registered and certified facilities will accept only personal funds for payments. Medicare and Medical Assistance do not pay for these facilities. Rent and other charges are self-pay. The manager can assist with financial questions. Be sure to ask if there is a "care charge" for residents needing assistance.
- Community Based Residential Facility (CBRF)
A home or apartment type setting where five or more unrelated adults live together. The goal of the CBRF is to assist individuals in achieving the highest level of independence of which they are capable. Different populations are targeted by the CBRF and some of these populations include: elderly, Alzheimer's, emotionally and mentally disturbed, developmentally and physically disabled and veterans. A CBRF is classified according to its size and the population it serves.
Payments for care can come from a variety of resources including personal funds and public funding if a person is eligible. The manager at the CBRF can assist with financial questions.
All CBRF's are licensed and monitored by the state. You can contact the Wisconsin Board on Aging and Long Term Care at (800) 242-1060 to determine the status of the facility that interests you, or you can visit the Bureau of Quality Assurance Website

An elderly person living alone at home can activate an emergency response system when a crisis happens and he/she is not able to get to a telephone (such as after a fall). He/she activates the system by pressing a button that is worn around the neck on a cord, or on a bracelet. The alarm is received at the hospital, who then calls the person on the telephone. If there is no response to the phone call, the hospital personnel call the elderly person's on file emergency responders. Responders may be neighbors, children or acquaintances who live in the elderly person's neighborhood.

Durable/Medical equipment is available to the public for private use in Manitowoc County. Refundable security deposit (check/cash only) required for each item. Loans may be short term in duration. A loan agreement must be signed at the time of the loan. Items must be returned in the same condition they were received at. Items available on a first come first serve basis.
