2022-2024 Aging Plan Community Input Survey (Please note this survey is for those 60 or older.)

We Value Your Input: Aging Plans are a requirement of each Aging and Disability Resource Center.  Every three years a new plan is developed.  We are in our current planning year. The deadline for our 2022-2024 Aging Plan will be October of 2021. Each plan includes goals based on the needs of the community in our service areas which include: Information and Assistance, Disability Benefits, Elderly Benefits, Caregiver Needs, Dementia Services, Nutrition, Transportation, and Prevention Programs.  Please complete our survey to have a voice in the planning of the Aging Plan.

ADRC Response to Coronavirus Emergency

Our physical office location is open to the public with safety protocols.  We will be maintaining social distancing throughout the building.  We will only allow a limited number of people in our waiting room at any given time.  Mask wearing is encouraged.  Phone appointments are also encouraged.  If you do not feel comfortable to come to our office, our staff is available to assist you by phone or video options. A dropbox is also available 24 hours a day outside our office for any paperwork that you may need to get to one of our staff members. The health and safety of our customers, volunteers, and staff is of utmost concern.  We are working with Kewaunee and Manitowoc County Public Health and are following CDC Guidelines.  In response to the current emergency:

  • All in person group activities and classes are cancelled through March 31, 2021.
  • Homebound Meal Delivery (Meals on Wheels) will continue with added precautionary measures
  • Congregate Dining Sites continue to be closed
  • Virtual Support Groups are available
  • ADRC staff is available to assist customers.

We will monitor this situation on a daily basis.  Updates will be put on our website and Facebook page and we will continue to send press releases to our local media.

The mission of the Aging & Disability Resource Center of the Lakeshore is to be a single source of information, assistance, and access to community resources for the aging population, people with disabilities, their families, and caregivers in the Kewaunee and Manitowoc County communities. 

To provide older adults and people with physical or developmental/intellectual disabilities the resources needed to live with dignity and security, and achieve maximum independence and quality of life.  The goal of the Aging and Disability Resource Center is to empower individuals to make informed choices and to streamline access to the right and appropriate services and supports.
